I am enclosing____________videotapes. Please transfer them to DVD and make____________additional sets of DVDs. I have included specific instructions on the back of this form or on enclosed pages.
  Pricing for the first copy is based on three factors:.
(a) The total time recorded to each DVD, i.e. $5.00/hour in 1/2 hour increments.
(b) The number of tapes copied to a DVD, i.e, $3.50 for each source tape.
(c) $1.50/blank DVD.

Pricing, additional copies (up to 10) are based only on length:
1.0 - 2.0 hrs, $10.00 ea.
less than 1 hr, $8.00 ea.

Inquire about pricing on large numbers of copies.

I understand that I will be charged $13.00 for return shipping to my address in the continental USA.
